Job Overview:

Responsible for planning, managing, implementing, and evaluating the daily operations of the Department of Case Management which includes oversight of effective resources utilization and the provision of high quality and excellent care. Work in collaboration with CM Director, administration and physician leadership to establish measurable standards and outcomes. Oversee nurse case management and social work services. Provide education to the frontline CM team; Conduct quality monitoring of staff productivity and documentation to ensure compliance with regulatory requirements; Conduct reviews and follow up of patient experience concerns to ensure high quality of care; Ensure continued development of the program to improve site performance of the CM Department and to enhance provision of case management services.

This is a full-time position responsible for the oversight of CM activities on the weekends and requires a schedule inclusive of weekends each week.

Qualifications:

Required:

  • If RN, BSN required, and master s degree in Nursing, Social Work, Healthcare Management, or comparable field preferred.

  • If Social Worker, MSW required; LCSW preferred.

  • Minimum 3 years clinical experience in hospital, home health, or community setting

  • Minimum 2 years experience in acute care case management.

  • Minimum 1 year supervisory experience is required

  • Current and valid license to practice as a Registered Nurse OR Licensed Social Worker in the state of New Jersey.

  • Certification in Case Management (ACM or CCM) strongly preferred/required within 1 year of hire.

  • This is a full-time position responsible for the oversight of CM activities on the weekends and requires a schedule inclusive of weekends each week

Essential Functions:

  • Management responsibilitiesincluding effectively supervises case management department activities, ensuring customer and stakeholder satisfaction; oversees weekend operations, including staffing, scheduling, and assignments and patient escalations, ensures revenue cycle activities are carried out on weekends, keeps administration appropriately advised on issues in an appropriate timeframe.

  • Clinical Excellence & Patient Care responsibilitiesincluding effectively participates in organizational performance and quality improvement activities, supports an effective quality improvement program with measurable, pertinent and realistic performance criteria to monitor and improve department services, promotes practice standards of Case Management and Social Work, ensures appropriate departmental operations for care coordination through psychosocial assessment, multidisciplinary rounds and patient advocacy; facilitates and reports activities related to revenue cycle, length of stay, and other key performance indicators including audits, variance tracking and financial impact.

  • Staff Development / Education responsibilitiesincluding promotes departmental services, activities, and programs through appropriate mechanisms; communicates and educates staff and appropriate healthcare team members on the plan of care and post-acute transition options.

  • Operational Excellence & Complianceresponsibilities including collaborates with healthcare team members, other members of department leadership, CM Manager, CM Director, and Senior Leadership Team to create processes that improve patient outcome while conserving hospital resources and limiting insurance denials; provides oversight for ongoing review and open and closed medical records to ensure compliance with all applicable government and commercial agencies for reimbursement; facilitates process standardization to promote improved outcomes, maximize resource utilization, and improve organizational efficiencies; and gathers and synthesizes data related to weekend operations and communicates trends to department and hospital leadership. Participates in strategic planning for 24/7 case management operations.
