Primary Health Care (PHC) was founded in 1981 by Dr. Bery Engebretsen in Des Moines, IA. Our mission has remained unchanged since that time, to provide healthcare and supportive services to all, regardless of insurance, immigration status, or ability to pay.
Based on the needs of the communities we serve, PHC offers a spectrum of medical and dental services including family practice, behavioral health, HIV care and services, and pharmacy. PHC’s Homeless Support Services is the entry point for serving people experiencing homelessness in Polk County. Enabling services are available to help patients with benefits enrollment, case management, transportation, translation, and patient education. We currently have locations in Ames, Des Moines, & Marshalltown.
As a Value Based Care Program Manager , you will serve as a subject matter expert in value-based reimbursement programs. This position is responsible for data collection and analysis utilizing data related to the value based care program. VBC Program Manager identifies trends and provides feedback to improve or resolve issues related to processes and data integrity. Coordinates with PHC team members to improve patient outcome achievement. Demonstrates iCare values in daily work.

What You Will Do
Utilizes health information technology applications to extract date for tracking, trending and analyzing health center performance and quality care metrics, payer incentive programs, VBC relationships, such as payer partners.
Supports initiatives related to Accountable Care Organizations (ACOs) and other VBC relationships, such as payer partners.
Educate new and existing staff on VBC and ACO requirements. Leads formal and informal training and information sessions for a variety of audiences.
Builds effective working relationships with PHC team members and external partners. Represents PHC on committees and workgroups related to value-based care.
Produces quality metric reports and datasets for internal customers to support improvements initiatives. Collects, organizes, edits, and distributes actionable data, which may include the use of data summary reports, dashboards, and other reporting tools to support VBC and population health agreements.
Completes manual chart audits and data validation. Maintains applications and audits registry data. Recommends improvements for the data capture process.
Assists in setting up the population management system for new tracking activities.
Makes recommendations regarding process improvement activities to improve VBC outcomes. Facilitates and follows up on quality improvement initiatives related to VBC contracts. Implements plans to meet identified program targets.
Assists clinics and programs with documentation, workflow, data entry, and data extraction related to VBC. Utilizes key HIT systems but not limited to population health system, data warehouse, EHR, government and grant reporting systems, payer portals and analytic tools.
Maintains knowledge of multiple regulatory agencies that drive data needs for PHC, including standard measure specifications such as HEIDIS, CMS, UDS, and Value Based Care contract measures and reporting requirements. .

Qualifications You Need to Bring
Required:
Bachelor's degree in health information technology, healthcare, business, or related field or equivalent combination of education and experience.
A minimum of 2 years of related work experience.
Effective verbal and written communication skills.
Proficient computer skills, including the use of Microsoft Office applications.
Willingness and ability to learn health information technology.
Organization, prioritization, and time management skills with ability to manage multiple priorities in a high volume environment.
Detail oriented with ability to and work independently.
Professional with ability to maintain calm in all situations; ability to show empathy while maintaining professional boundaries.
Excellent interpersonal skills with ability to establish and maintain effective working relationships.
Preferred:
Experience with effectively implementing projects and initiatives with staff at all levels of an organization, and/or with external stakeholders and community partners.
Community health center experience.
Master's degree in a related field.

Your experience and skills determine your base pay. The hiring range for this position is typically $47,120 - $58,900 annually. Candidates with extensive work experience related to this position may be considered for additional compensation up to the pay grade maximum of $70,680 annually .
