NineLine Veteran Services Outreach Coordinator in Fife, Washington
NineLine Veteran Services Outreach coordinator QRF Program Full-Time or Part-Time Travel Position -- Multi county region -- travel is a must for this position. This position will report to the Outreach Manager and Program Director. It will provide critical linkages for clients beginning at their intake and assessment, following through to their stabilization through appropriate community support. These positions assist eligible veterans and their family members and provide case management support, advocacy, and wraparound services support including services with the VA and the community. Experience in supportive services to veterans preferred. ESSENTIAL JOB FUNCTIONS: Perform outreach to veterans and their families • Coordinate outreach efforts for the Veterans Program, including but not limited to establishing an outreach plan to target the three categories of eligible participants as outlined in the Staff Sergeant Parker Gordon Fox Suicide Prevention Grant Program, program guide, develop a set schedule to visit the VA, clinics, shelters, and other places that veteran families are likely to congregate, and develop communication/ updates with the VA and other community providers. • Screen potential program participants for program eligibility using VA-approved forms and follow up with applicable agencies to verify veteran and income status information. • Provide the following services: (1) Outreach to identify those at risk of suicide (2) Baseline Mental Health Screening (Participants over 18 years of age) (3) Education on suicide risk and prevention to families and communities (4) Referral to clinical services for emergency treatment (5) Case management services (6) Peer support services (7) VA benefits assistance for eligible individuals and their families (8) Assistance with obtaining and coordinating other benefits provided by the federal government, a state or local government, or an eligible entity (9) Assistance with emergent needs relating to health care services, daily living services, personal financial planning, and counseling, transportation services, temporary income support services, fiduciary and representative payee services, legal services to assist the eligible individual with issues that may contribute to the risk of suicide, and childcare. (10) Nontraditional and innovative approaches and treatment practices, as determined appropriate by the VA. (11) Other services necessary for improving the mental health status and wellbeing and reducing the suicide risk of eligible individuals and their families as determined appropriate by the VA. • Provide or arrange for support and wraparound services, including connections to all eligible VA and mainstream resources to support. • Communicate effectively (under confidentiality rules) with the client's service team to rapidly address and correct issues with services or supports - especially living skills, employment, education, family, and socialization. • Participate in program and professional supervision, attend required staff and Project Management Team meetings and trainings, and contribute to team-based collaborative planning. • Complete other duties as needed and requested.
