Wellspring Lutheran Services Hospice Nurse (RN) in Frankenmuth, Michigan

Hello! Thank you for your interest in joining the Wellspring team as a Full Time Hospice Nurse with Wellspring. As a Hospice Nurse with Wellspring, you'll be part of a team of nurses, CNAs, and administrative professionals who are committed to supporting our staff in their work of serving seniors and their families within the Saginaw, Frankenmuth and surrounding areas. In this position, you must be currently licensed by the State of Michigan Board of Nursing as a Registered Nurse and successful completion of an accredited Registered Nursing program. Two years of nursing experience with current home health/hospice, medical, surgical, or critical care experience preferred.      

*As a Hospice Nurse, you will: *      

  • Seek and obtain sufficient information necessary to function effectively in all hospice settings where assigned

  • Provide professional nursing care and comfort by utilizing all elements of nursing process

  • Assess and evaluate patient and family status by:

    • Providing the initial patient assessment

    • Reviewing and implementing the Plan of Care to ensure pain and symptom relief

    • Regularly re-evaluating patient/family needs to ensure comfort, relief and hope

    • Revising the services delivery plan of care as necessary

    • Participating in the interdisciplinary group meetings

  • Administer medications and treatments as prescribed by the physician

  • Initiate and apply appropriate preventative, therapeutic and rehabilitative nursing procedures and techniques; maintain communication and coordination regarding patient care with the physician, supervisory personnel, and other disciplines

  • Complete, maintain and submit accurate and relevant clinical notes regarding patient's condition and care given according to procedure

  • Supervision of hospice aides every 2 weeks
