RATES:
MON-FRI Day $26.26/hr (base), Evening $28.36/hr (base+8%), Night $29.41/hr (base+12%)
SAT-SUN Day $26.79/hr (base+2%), Evening $28.89/hr (base+10%), $29.94/hr (base+14%)

Summary: The Licensed Nurse (LPN) is responsible for managing the residents existing care plans and supervising the day-to-day resident care activities during the shift. Both care management and supervisory responsibilities must be executed in accordance with state and federal regulations and facility policies and procedures.
Responsibilities include the following.
Direct day-to-day resident care activities.
Ensure that daily licensed nursing functions are carried out in accordance with state and federal regulations,
and facility policies and procedures.
Cooperate with other departments in the facility when coordinating resident care services to ensure that residents’ total regimen is maintained.
Report resident needs to next shift RN Supervisor .
Supervise the CNA’s delivery of care and proper implementation of the standard of care. Report any ongoing trends to the Clinical Nurse Manager/Nursing Supervisor.
Complete medical forms, reports, evaluations, studies, charting, care plans etc. as needed. Attends care plan meetings as needed.
Meet with interdisciplinary team members as needed to plan and coordinate shift services and programs.
Admit, transfer, and discharge residents.

Supervisory Responsibilities
Directly supervise Certified Nursing Assistants employees in accordance with the organization's policies and all applicable laws. Plan, assign, and direct work, as well as address complaints and resolve problems.
Education and/or Experience
One year certificate from college or technical school for Nursing required.
Minimum one year of long term care or acute care experience required.
Valid NYS LPN license and CPR required.
