Clinical Coordinator at Elara Caring

Schedule: Monday through Friday, 8:00a to 5:00p

Your Role as a Clinical Coordinator
As a Clinical Coordinator, you’ll be the heartbeat of our operations , ensuring that our teams deliver the highest standard of care. You’ll play a critical role in:
Team Building : Assemble and inspire a team dedicated to enriching patients’ lives.
Operational Excellence : Keep our systems organized, efficient, and optimized for seamless care delivery.
Workflow Innovation : Identify and implement strategies to improve productivity and streamline processes.
Support & Communication : Be a liaison for patients, families, and staff, fostering positive and supportive interactions.
Preventative Care : Take proactive measures to reduce hospitalizations and ER visits, ensuring patients stay where they thrive—at home.
Your Day-to-Day Will Include :
Organizing patient care records and assisting with utilization reviews.
Supporting clinical supervisors with non-clinical inquiries.
Managing inventory, equipment, and departmental communication.
Ensuring compliance with HIPAA and maintaining confidentiality.
Promoting Elara Caring’s mission and philosophy in all interactions.
What You Bring to the Table
At least 1 year of experience in hospice or home healthcare.
Strong knowledge of medical terminology and proficiency with office equipment.
Familiarity with Medicare and Medicaid guidelines (preferred).
A commitment to compassionate care and effective communication.
Minimum of High School Diploma, college degree preferred
Highly preferred to have experience with Microsoft Office Suite
