The Practice Support Unit Nurse Practitioner (PSU) will work in the Tampa Bay area contracted primary care market to support quality risk adjustment coding and screening needs. Patients may be seen in a clinic, long term care facility or in the patient’s home as part of a collaborative team that may include, but is not limited to, Family Practice and/or Internal Medicine Physicians, RN Case Managers, and clinic staff. The purpose of this collaborative team is to provide a comprehensive, coordinated model of care to a mostly Medicare-aged population in either the outpatient and/or inpatient setting. The NP may assist with new primary care physician (PCP) onboarding and training to the PSU medical model as well as provide insight into trending the support needs of the practice thereafter. The NP performance will be tracked and comparatively reported using metrics related to the strategic direction and operational needs of the organization.
Essential Job Functions:
The NP is expected to be able to provide care consistent with their licensure and/or certification and within the scope of their role in WellMed. All services performed and referred for, are limited to those core services as defined within the department.
Performs history and physical exams with medication reconciliation
Risk adjustment coding and screening in the member’s PCP office, skilled nursing facilities and in patient homes
Participate in wellness launches and community events
Identify barriers to accessing care and medication adherence and utilize available resources to assist patient compliance
Counsels patients on various disease conditions they may be treating to ensure that the patient is educated to their disease process, treatment, and expected outcomes
Provide coverage in WellMed owned primary care clinics when necessary
Place referrals for annual wellness testing as appropriate
Completion and closure of HEDIS and FOC gaps
Interdepartmental collaboration to address patient needs
Provide contracted provider education and support, supporting PCP/Patient relationship
Perform chart audits
Participate in unit team building and new initiatives
Document in an (EMR) electronic medical record
Will be required to travel throughout their assigned market, with occasional travel outside of designated market for coverage as r.
Will be required to have reliable transportation and maintain insurance on the vehicle
Diagnoses and treats both acute and chronic conditions such as diabetes, hypertension, kidney disease and other chronic medical conditions. The NP will also treat acute conditions such as minor injuries and other acute medical conditions such as infections or exacerbations of chronic diseases
Orders and interprets diagnostic lab work, x-rays, EKG
Prescribes medications and other modalities of treatment as needed to treat those conditions listed above
Works as an integral part of a comprehensive and collaborative team in order to provide quality evidenced-based care to our patients
Performs all other related duties as assigned
Required Qualifications:
Masters of Science in Nursing
Certification through the American Academy of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC) in Adult or Family Medicine
Current licensure through the Florida Board of Nursing
Valid DEA license or the ability to obtain prior to employment
Current Basic Life Support (BLS) certification
Comfortable managing a primarily Medicare-aged patient population
Reliable transportation and maintain insurance on the vehicle
Preferred Qualifications:
1+ year of post-grad clinical experience as a primary care Nurse Practitioner
Risk Adjustment coding experience within the healthcare industry
Basic knowledge of HEDIS and FOC measures.
Bilingual (English/Spanish) proficiency
