Title: SSVF (Supportive Services for Veteran Families) Case Manager Job Description: Abilities Required: - Must be proficient with computers - Must be able to communicate clearly and professionally both in the spoken and written word - Strong ability to work as a team as independently - Must possess organizational, planning, information gathering, information monitoring, problem analysis and problem-solving skills - Must possess judgment and decision-making ability, initiative, confidentiality, and attention to detail and accuracy - Must have Associates or Bachelors Degree - Valid NYS Drivers License
Responsibilities: - Complete pre-screens on all Veterans that seek assistance. - Conduct community outreach - Work with other agencies to ensure Veterans in need are being reached. - Compile data for monthly reports - Complete and file all applicable reports provide direct client services - Perform eligibility assessments to determine that Veterans are eligible for SSVF Assistance determined by the CNYVOC, that the Veteran is homeless or at imminent risk. - Provide holistic and comprehensive case management services to all clients, with a strong focus on SSVF tenants: including intake assessment, benefit assessment, goal setting, long term case management, tenant education, advocacy, and referrals. - Assist the Housing Director with the maintenance of the housing binders, tenant leases, and other paperwork - Assist the Housing Director in monthly housing inspections - Identify opportunities for collaborative ventures with local apartment complexes, rental agencies, and owners of rental houses to begin the process of finding local affordable housing for Veterans. - Ensures all applicable regulations and guidelines are followed before requesting approval for assistance. - Connect families with needed and available community resources. Follow-up with clients and agencies as appropriate to document use/success of referral. - Help all Veterans and their families that are in need. - Attend community-based meetings when needed. - Work closely with the VA, DOL and any other government agencies and community agencies as needed. - Complete SOAR applications for eligible clients - Input HMIS data
