PRIMARY FUNCTION: The practice transformation coordinator's primary role is to support assigned practices as they work towards improved quality of care for patients and practice transformation. As a part of the team the individual will be responsible for supporting projects and initiatives from development to completion. ESSENTIAL FUNCTIONS:
- Facilitation and coaching of practice redesign with regard to quality and health outcomes including the management of PDSA cycles, quality improvement projects, monthly outreach campaigns, and future pathway projects.
- Education physicians and/or staff EHR efficiencies and other software in various waysincluding support for medical students and participation (presentations) at a variety of meetings.
- Identifies opportunities for improvement of processes and flows and design documentation and organize roll-out of best practicesthrough patient outreach, reports, chart reviews, core quality initiatives, and data evaluation.
- Provides outreach and serves as liaison to provider organizations and physician practices, builds relationships with provider organizations including the central population health team. Includes attending meetings, elbow to elbow teaching, presentation of new processes and monitoring for barriers.
- Works with Population Health and department staff to create and update web-communications and informationincluding using Microsoft programs (Teams, PowerPoint, Excel, Word, Outlook, etc.) and attending required meetings.
- Other tasks as assignedincluding but not limited to participation in the interview process for new hires.
- Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
OTHER FUNCTIONS AND COMPETENCIES needed in addition to Essential Functions:
* Provide project management support including generating data, data analysis, process mapping, communication, organization, resources, presentations, and other support as needed.
* Provide education and support regarding the use of EPIC, Qlik, and other applications.
* Identify areas of improvement and using data to drive improvement.
* Create educational documents, process mapping, and supportive information/resources for employees and orgainizing the distribution and explanation of the information through presentations, educational sessions, and elbow to elbow learning.
* Attend meetings as required as a representative of the department and disseminating information received during those meetings.
* Creating and maintaining presentation materials, updated information in all settings.
EDUCATIONAL/TRAINING REQUIREMENTS:
High School Diploma, required
Bachelor's Degree, preferred
